About Our Location
We are located at 435 W. Oak Street Suite G in beautiful Cottage Grove, Wisconsin. Our office space is on the far right side of the building. Our near by neighbors include Monona State Bank, Bank of Sun Prairie, McDonald's, Piggly Wiggly, and many others. Our parking lot has plenty of spots available and numerous spots available for our patients with valid state issued disabled parking license plates or placards.

- Proceed East Down Cottage Grove Road / County Highway BB Towards Cottage Grove.
- Continue down Cottage Grove Road / County Highway BB until you enter Cottage Grove.
- Once you are in Cottage Grove turn left on W. Oak Street.
- Our office is located on the left side of the road.
- Leave Deerfield by going North on Highway 73
- Make a left turn onto County Highway BB
- Continue down Highway BB for approximately 7.5 miles.
- Once you are in Cottage Grove turn right on W. Oak Street.
- Our office is located on the left side of the road.
Option #1
- Leave Sun Prairie by traveling South down County Highway N.
- Make a right turn onto County Highway N and continue down Highway N for approximately 5.2 miles. (You will pass under Interstate 94 after 6.7 miles.)
- Make a right turn onto Cottage Grove Road / County Highway BB and drive for approximately 0.5 miles.
- Once you are in Cottage Grove turn right on W. Oak Street.
- Our office is located on the left side of the road.
- Leave Sun Prairie by traveling South on US Highway 151.
- After 3.6 miles take 97A for I-39 S/I-90 E/I-94 E toward Chicago/Milwaukee
- Proceed East on Interstate 94 for approximatly 7 miles.
- Take Exit 244 to Cottage Grove Road
- Proceed South on County Highway N for approximately 1.5 miles.
- Make a right turn onto Cottage Grove Road / County Highway BB and drive for approximately 0.5 miles.
- Once you are in Cottage Grove turn right on W. Oak Street.
- Our office is located on the left side of the road.

- Leave Marshall by traveling South-West on County Road TT. Proceed on TT for approximately 8.2 miles.
- Once you arrive at the around about proceed around the around about until you can turn south on County Highway N. Proceed on N for 1.5 miles.
- Make a right turn onto Cottage Grove Road / County Highway BB and drive for approximately 0.5 miles.
- Once you are in Cottage Grove turn right on W. Oak Street.
- Our office is located on the left side of the road.
- Leave Marshall by traveling South on County Highway 73 for 4.2 miles.
- Take the Interstate 94 West entrance ramp West for 6 miles.
- Take Exit 244 to Cottage Grove / Highway N
- Proceed South on County Highway N for approximately 1.5 miles.
- Make a right turn onto Cottage Grove Road / County Highway BB and drive for approximately 0.5 miles.
- Once you are in Cottage Grove turn right on W. Oak Street.

- Leave Monona by taking Monona Drive North to Cottage Grove Road
- Proceed East Down Cottage Grove Road / County Highway BB Towards Cottage Grove.

- Leave McFarland by traveling North on Stoughton Road / Highway 51.
- Proceed on Stoughton Road until you reach Cottage Grove Road.
- Take a Right Turn on Cottage Grove Road / County Highway BB Towards Cottage Grove.
- Continue down Cottage Grove Road / County Highway BB until you enter Cottage Grove.
- Once you are in Cottage Grove turn left on W. Oak Street.

- Leave Waterloo by traveling South West on WI-19 for 4.4 miles.
- Proceed on Stoughton Road until you reach Cottage Grove Road.
- Turn left onto Wisconsin 73 South / Deerfield Road / Highway 73 South and Continue to follow Wisconsin 73 South / Highway 73 South
- Take Interstate 94 West for 6 miles.
- Take Exit 244 to County Highway N / Cottage Grove.
- Proceed South on County Highway N for approximately 1.5 miles.
- Take a Right Turn on Cottage Grove Road / County Highway BB Towards Cottage Grove.
